Conditions in and around an excavation can change quickly. What was safe at the start of the day may become deadly after rain, vibrations, or nearby work. A missed inspection could mean missing signs of an imminent collapse, hazardous atmosphere, or water accumulation.

- Significant vibrations (heavy equipment, nearby traffic)
- Changes in soil conditions or trench configuration
- The trench must be free of water, loose soil, or unprotected edges.

- Findings must be documented and clearly communicated to the crew before work begins.
