Incident Reporting and Investigation

Toolbox Talk:

Reporting Near Misses Is Just As Important

June 27, 2025

Near misses are warning signs. If something almost caused an injury, it’s only a matter of time before it actually does.

Why It Matters:

Many serious incidents are preceded by near misses that went unreported. By speaking up, you help fix problems before someone gets hurt — maybe even yourself. Reporting near misses builds a proactive safety culture and gives leadership the data to make better decisions.

Key Points:

  • A near miss is a free lesson. Learn from it and share it.
  • Report it just like you would an injury. Same urgency, same process.
  • You won’t get in trouble. Reporting helps everyone, and we expect it.
  • Fix what you can immediately. Don’t leave the hazard for someone else.
  • Follow up. Make sure the hazard doesn’t come back.

✅ A close call today could be a tragedy tomorrow — unless you speak up.

Ask the Crew:

  • When was the last time we had a near miss?
  • Did we report it and correct it?
  • Would you feel comfortable telling someone if it happened again?
