Step-by-Step Lockout/Tagout Procedures Save Lives

Every piece of equipment must be de-energized, locked, and verified before any maintenance or servicing begins. Following a consistent, step-by-step Lockout/Tagout (LOTO) procedure ensures no hazardous energy is accidentally released.

Why It Matters:

Improper or skipped steps in the LOTO process can lead to unexpected energization, electrical shock, crushing, or amputation. OSHA requires specific steps under 1910.147 and 1926.417 to ensure workers are protected.

Key Points:

  1. Notify all affected employees before starting LOTO.
  2. Shut down the equipment using normal procedures.
  3. Isolate all energy sources (electrical, hydraulic, pneumatic, thermal, etc.).
  4. Apply lockout devices and tags to each energy-isolating device.
  5. Release any stored energy (e.g., bleed air, discharge capacitors, block moving parts).
  6. Verify isolation by trying to start the equipment or using appropriate testing devices.
  7. Perform the work safely.
  8. When work is complete:
    • Remove tools and reinstall guards.
    • Ensure all workers are clear.
    • Remove LOTO devices only after confirming it’s safe.
